Community Living Service Delivery (CLSD)

Information:

Community Living Service Delivery (CLSD) is a branch of the Income Assistance and Disability Services Division for the Ministry of Social Services. The mission of CLSD is to support the development of inclusive communities for individuals with intellectual disabilities. Employees of CLSD are committed to working with their clients to define their goals, dreams and aspirations, and to help enhance their development and quality of life

Programs:

Residential Services: Community Living Service Delivery provide a range of residential services and day programs based on an individual’s support need, including:
• Group homes – homes that are staffed to provide personal care, supervision and support for adults with intellectual disabilities. They are located in residential neighbourhoods throughout the province.
• Group living homes – individuals who share a group living home are responsible for paying their basic shelter costs. CLSD provides funds for the support staff that may be required.
• Supportive living programs – provide adults living in their own apartments with the limited support and supervision they may require to live as independently as possible.

Community Case Management– Each CLSD client is assigned a Community Service Worker that can provide a broad range of support including:
• Individual client support, counselling and crisis intervention.
• Assessment of client and family needs
• Family support services such as counselling, respite planning, referrals and providing other information as needed.
• Development and support of approved private-service homes
• Coordination of planning and services with individuals, families, community-based organizations
and other stakeholders.

We hope to provide holistic support services to individuals with disabilities to develop as participatory citizens.

Program Information:

  • Day Program– activities in Shaunavon and surrounding communities and in the vocational centre for individuals experiencing disabilities. Includes and activity area, and a workshop which functions 9:00 am until 4:00 pm Monday-Friday.
  • Supported Employment- is an employment services where job coaching, development, and employments skills are built for individuals experiencing barriers to employment and/or disability.
  • Group Homes- 24 hour care and supervision to individuals experiencing disabilities in Shaunavon and area.
  • Group Living Homes– assists individuals experiencing disabilities to live in their own home, with limited supervision.
  • Shaunavon Van for the Disabled- Provides accessible transit services in Shaunavon and district.
  • Supportive Living Services– provides residential supports as determined by a person centred plan. Services may include meal planning and preparation, cleaning and managing a home, use of leisure time, accessing community resources and support networks, assistance with budgeting, and shopping and money management skills.
  • SARCAN– beverage and electronics recycling (Monday-Friday 9:30 am tunil 4:30 pm in Shaunavon).
  • Eco-Centre- Oil, oil filter, oil container and antifreeze collection.

Fresh Start- JP’s Place

Information:

In 2013, Fresh Start launched JP’s Place, an initiative to address the issue of homelessness in our community. A beautiful home was purchased and is available to young men between 15-17 years of age in need of same. JP’s Place is Swift Current’s first peer home.

JP’s Place offers the spaciousness of 1700 square feet and the opportunity for young males to live in a safe, secure setting. Our cozy, comfortable home provides youth with all the basics and more. Each youth will have their own fully furnished bedroom, as well as daily wholesome, nutritious meals. JP’s Place is fully supported by qualified staff.

Our goal is to create a home that offers the grace that all deserve.

Saskatchewan Housing Corporation (SHC) provides a range of programs and services to help Saskatchewan people in greatest housing need.

SHC Life Lease Program: provides moderate-income senior individuals and couples with affordable housing in exchange for a deposit and a monthly occupancy fee.

Rental Housing for People with Low Incomes:  The Social Housing Program provides safe and adequate housing to families, seniors, and people with disabilities who have low incomes.

Rental Housing for Seniors in Small Communities:  provides rental housing to individuals and couples who are 60 years of age or older. This program is offered in small communities where there are no eligible applicants for Social Housing Program units in the community.

The Personal Care Home Benefit (PCHB) provides seniors with monthly financial assistance to help them with the cost of living in a licensed personal care home.

Information:

The Saskatchewan Rental Housing Supplement (SRHS) is a monthly payment that helps families with low-to-moderate income, and individuals with disabilities, access quality and affordable rental housing. The supplement is made up of two benefits:

  1. The Family Rental Housing Supplement – provides benefits for families with low-to-moderate income who have children under the age of 18 to help pay their household rental costs.
  2. The Disability Rental Housing Supplement – provides benefits for low-to-moderate income single individuals, families, and couples without children who have supports in their household to accommodate a physical or cognitive disability-related need

The Housing complex in Swift Current is located just south of the Salvation Army in the 700 block of 1st Ave. N.W.

General Requirements

Generally, people are eligible for housing if their household income falls below an amount set by Canada Mortgage and Housing Corporation (CMHC). Silver Sage Housing Corporation will assess each applicant’s need for housing based on criteria which includes the applicant’s income, current living situation and personal and family requirements. This ensures that priority is given to households in the greatest need.

To qualify for subsidized housing, you must meet all of the following criteria:

Affordable housing is available for families, seniors, students, and people with disabilities who can live independently and qualify for a disability pension, or who cannot work because of a disability. Housing is targeted to persons with low to moderate income.

You must be at least 19 years of age.  Some or all of your annual income comes from employment and you file an annual income tax return.

 Who is Not Eligible:

If you own your own home or if you own a home but do not live in it.

  • Community Based Day Program (Monday-Friday 8:30 am until 4:00 pm)
  • Southwest Apartments- Affordable housing for individuals with an intellectual disability or mental health issue.

Referral Process

All applicants must meet the criteria of Community Living Service Delivery (CLSD) to be a part of any of our programs within the exception of being a tenant in the apartment building. Applications can be sent through email or picked up at our main office which is located at 440 South Railway Street East.
