City of Swift Current – Families in the Park

Information:

Our summer program runs in July and August at Kin Park. In partnership with the Family Resource Centre, Swift Current Library, ECIP, Salvation Army and SW Literacy Committee we aim to create an atmosphere through our park program that encourages families to stay active while spending quality time together. Through the contribution and involvement of your agency within the program, families within the community are made aware of the wonderful resources our park program has to offer. Last summer we had six theme weeks such as Movin’ and Groovin, Art Attack, Under the Creek, Out and About, ect.

City of Swift Current – Groovin’ in the Park

Information:

This is a free fitness program for women that promotes the physical, emotional, and social benefits related to health, fitness, and physical activity while enjoying the outdoors.

This program is offered in the spring to women of all ages and fitness levels on Tuesday mornings from 9:00 am until 10:00 am and Thursday evenings from 7:00 pm until 8:00 pm at Elmwood Park from May 23 to June 29, 2017 An added service of groovin’ in the park is free child supervision during the morning program.

Referral Process

Parents are required to register their children for the entire summer. There will be other permissions forms which will be circulated during the program to provide permission for field trips outside of Swift Current.  Please note there are no limits for attendees allowed to participate, as everyone is welcome!

The program runs Monday to Friday from 2:00 pm – 8:30 pm at “The Centre” (55 1st ave NE Swift Current, SK).  It is a free drop-in program that is filled with cooking classes, sport clinics, board games tournaments, and field trips!

Referral Process

Cypress Health Region – Chronic Disease Management (CDM)

Information:

Healthy living focuses on prevention, the management of chronic conditions, encouraging support for self-care and the understanding that factors outside of the health system can influence individual and community health. It is about keeping you well, rather than just treating you after you have become sick. As the Cypress Health Region works toward bringing a primary health care approach to more and more communities, you may see some changes in how you access care. One of these changes is a more deliberate, formalized health care team approach. This approach gives you access to a team of health professionals, including some you see already (like a family doctor or nurse practitioner) and maybe some you haven’t considered before (certified diabetic educator, pharmacist, physiotherapist, midwife, health educator, or dietitian).You are not alone, let’s work together toward a healthy lifestyle and manage chronic conditions effectively

Cardiac Program

  • Group classes offered several times per year
  • Risk assessment identification – participants identify their heart hazards
  • Positive steps toward making healthy lifestyle choices
  • Gain knowledge of how the heart works, risk factors, common tests and procedures, activity, medications, heart healthy diet, stress management

  • One on one & group counseling
  • Label reading, portion distortion
  • Healthy eating and weight management
  • Diabetes, heart disease, cancer & other special medical needs
  • Normal nutrition throughout the life cycle

Diabetes Program

  • Individual counseling for inpatients and outpatients
  • Self-management counseling on blood glucose monitoring, nutrition, lifestyle issues, activity and medications
  • Includes type 1, type 2 and gestational diabetes

Live Well with Chronic Conditions

  • A self-management program developed by the Stanford University
  • Fun and practical group class that runs 2.5 hours once a week for 6 weeks
  • Build skills for managing your health and maintaining an active and fulfilling life
  • Practical suggestions and support to build confidence in coping with everyday challenges of any chronic condition
  • Self-management skills include: symptom management, communication skills, relaxation techniques, goal setting, problem solving

Pulmonary Rehabilitation

  • Offered twice a year spring and fall
  • Participants meet twice a week for 6 weeks—one hour supervised exercise training & one hour of education

Lower Limb Assessments

  • Lower leg and circulation assessment to check blood flow and sensation in feet. Circulation testing can identify the effects of clogged arteries and veins and prevent further problems.
  • Includes foot care education and lifestyle counselling.
  • People at risk include those with diabetes, those over 60 years of age, and those with a history of strokes and heart attacks.

Group classes on healthy lifestyle and chronic disease may include:

Referral Process

Referrals to Health and Well Being programs in the Cypress Health Region may be made by self, family, physician, nurse practitioner or other health care professional. When appropriate, referrals may be made from one health care professional to another. For more information on Health and Well Being Programs please contact the Primary Health Care department.

Cypress Health Region – Family Centre for Weight and Health

Information:

Obesity is a complex condition and is often the result of many factors, including lifestyle habits, genetics, sleep problems, and environments that do not promote optimal health. The Cypress Health Region is sponsoring a new clinic to help improve the health and well-being of children struggling with weight concerns. The goal of the Family Centre for Weight and Health is to help children feel happy about themselves, feel loved and accepted for who they are,  and to establish lifelong healthy eating and physical activity habits.

The Centre uses a team based approach to care that includes a pediatrician, registered dietitian, physical therapist, and a counsellor. Parental involvement is a prerequisite.

Children and youth may be referred by their family physician, pediatrician, nurse practitioner, or other health professional.

  • Recreation Therapy: Recreation therapy focuses on restoring leisure independence and the overall well-being of clients.
  • Speech Language Pathology: Speech language pathology focuses on clients who need help with communication or swallowing.

United Way Active Play

Information:

This program is offered a the six schools which run grades K-8 in the city of Swift Current for six weeks at each school, and twice a week for students grades 1-6. The goal of this program is to provide a safe, supervised program for children to be physically active and have fun while learning about healthy choices such as exercise, nutrition and wellness. The information is sent home at each school for the parents and caregivers to register their children.

Referral Process

Parents and guardians register their own children- please note the first 40 registered are guaranteed a spot in the program. If the school administration or staff recommend that a specific student would greatly benefit from this program, we make sure to get them into the program.
